[BUG] MCP tool calls fail with "No such tool available" error despite tools being registered

What's Wrong?
Description:
- Attempted to use chrome-devtools MCP tools (list_pages, take_screenshot, etc.)
- Tools show as available in tool registry
- Chrome is running with --remote-debugging-port=9222
- chrome-devtools MCP server initialized successfully
- All tool calls fail with "No such tool available" error
- Same MCP server works fine in Claude Desktop UI
- Error is consistent and reproducible

Open and functionality with Chrome MCP
