[Bug] Claude Code reliability degradation: unable to resolve bugs and introducing regressions

Bug Description
Claude code used to be on point last week, but something changed within the last 5 or so days, it cannot accomplish anything successfully. It cannot resolve bugs, and created several bugs I had to roll back a week. No idea what happened to the great product.
