[BUG] mobile app PR UI stuck showing closed PR doesn’t update across sessions

What's Wrong?

Title: Mobile app PR UI stuck on closed PR, doesn’t update across sessions
Description:
When using Claude Code mobile:

1. Push a branch, click the PR link from output, create PR on GitHub
2. Mobile UI shows “View PR” button linking to that PR
3. Merge/close the PR on GitHub
4. Bug: UI still shows the old closed PR link
5. Push a new branch in a new session - UI still shows the stale PR #67

Expected: UI should clear or update when PR is closed, or when working on a different branch
Environment: Claude Code mobile app, iOS

Steps to Reproduce

When using Claude Code mobile:
