[BUG] Can't run /compact command

What's Wrong?

Trying to run "/compact" and getting  Error: Error during compaction: Error: API Error: 400 {"type":"error","error":{"type":"invalid_request_error","message":"tools: Tool names must be unique."},"request_id":"req_011CWAQTSevYFRww13Vty1QQ"}, removed all mcp-servers

What Should Happen?

Command "/compact" should be finished and conversation should be continued
