[Bug] Claude stops responding after successful tool calls with "No response requested."

Bug Description
Bug Summary:

• After tool calls complete successfully (all hooks show "succeeded")
• Claude responds with "No response requested." and stops
• User has to prompt again to continue
• This happens repeatedly even after hook refactoring

Relevant Context:

• Hooks are completing successfully (PreToolUse, PostToolUse, Callback all show "succeeded")
• Global settings reduced to only PermissionRequest auto-approvals
• Project settings have minimal hooks (2-3 per project)
• 9 plugins enabled with internal "Callback" hooks

This happens with:

• Minimal hook configuration (only PermissionRequest auto-approvals)
• Plugin Callback hooks running
• Model: claude-opus-4-5-20251101

Workaround: Send follow-up message like "continue" or "ultrathink"
