[Bug] Incorrect base filepath resolution causing repeated token waste on failed operations

Bug Description
Claude consistently misspells the base filepath for all my projects and wastes incredible amounts of tokens retrying impossible things. This happens across many different projects, changing

• /Users/yaleman/Projects/<projectname>/blah blah to
• /Users/yaelman/Projects/<projectname>/blah blah (note the yael instead of yale)

from there it'll just keep spinning, attempting to access impossible paths over and over wasting my credits and hitting limits to the point where I lose access to my paid subscription.

The only workaround is hope to spot it while using the agent and reset the session and wait until next time it happens. I've even added a pretooluse hook that should check for it, but somehow it's bypassing that too today.

Surely there's some system-level things that can go "hey, that tool you keep trying to use is completely invalid, reconsider your life choices?" or something similar.
