[Feature Request] Improve Claude plan discovery with project-relative paths and descriptive naming

Bug Description
Feature request: discoverable/intuitive plan path

Currently, Claude plans are written to $HOME/.claude/plans/<random-name>.md

This is unintuitive since:

• location doesn't reveal which project it relates to
• filename doesn't indicate when the plan was made or what it relates to

Proposal:

• use more intuitive syntax, eg {projectPath}/.claude/plans/{date}-{description}.md
• or make it configurable
