[Bug] Opus model exhibiting poor agentic behavior and hallucinating tasks

Bug Description
with this new opus upgrade this is going off the rails constantly. Constantly doing things I didn't suggest and are often quite wrong. This is by definition the junior engineer who is all D-K. Voted most likely to get itself way off the rails on a thing it is the most wrong about.
