[BUG] IDE connection shows success but /status reports not connected

What's Wrong?

Running /ide command shows successful connection to VS Code (with checkmark ✓), but /status shows:

IDE: ✘ Not connected to Visual Studio Code

IDE integration features (file selection, diff viewer) don't work.

What Should Happen?

After /ide succeeds, /status should show connected and IDE features should work.

Steps to Reproduce

1. Open VS Code at /Users/xxx/Projects/xxxx
2. Launch claude in VS Code integrated terminal
3. Run /ide → Select "Visual Studio Code" → Press Enter
4. Run /status → Shows "Not connected"
