[BUG] Memory Instruction Retention Breaks in Consecutive Interactions

Bug Description
"#" memories created under ~/.claude are being completed ignored, especially within the same session. I explicitly make claude remember to never to create new scripts unless asked and it goes ahead and creates more scripts in a couple of prompts down the line. I've had Claude do this for other simpler instructions such as "# never restart the server / services yourself, tell me when you want me to and I'll do it manually" and a couple of seconds later it's restarting services itself - in fact writing elaborate scripts that will restart various docker containers.

Please note this is NOT 0.2.104 specific, it's been happening in 0.2.115 as well - I downgraded from 0.2.117 due to https://github.com/anthropics/claude-code/issues/1146
