Bug Description
/bug Plugin registration completely broken - installed_plugins.json never created, all plugins show as "No plugins installed" despite files existing in .claude\plugins\ directory. Trust prompt appears every launch from home directory even with workspaceTrustGranted:true. /doctor shows no plugin-related diagnostics.
