[BUG] Survey easy to accidentally answer

What's Wrong?

Its really common to start a prompt with 1. do x 2. do y but your survey is 1,2,3, I have multiple times accidentally marked something as bad when it was not bad by accident. you might need to do something like /1 so that people dont accidentally vote

make it hard to accidentally vote
