[Bug] Claude ignores file output directive and freezes on console input during plan generation

Bug Description
i asked claude to put a plan into a md and not to update any code.\
it ignors me, and create a plan in the console window, but offeres to allow me to edit in visual studio with a control something. I respond, no put it in the file and gave the file location.\
it does not, so i start typing a response in the console regarding the long plan.\
then the window freezes and i loose my long set of instructions.
