[Bug] BashOutput process state persists after /clear command

Bug Description
BashOutput does not clear killed and done processes after running the /clear command. I often use the /clear command to remove context and start fresh. When I use skills that use Bash and BashOutput (like long running agents), they pollute the context after I run the /clear command. I am forced to /exit and re-start claude.
