[Bug] Slash suggestions: duplicate IDs cause multiple selected rows and stale rendering

Bug Description
Slash suggestions: two entries with the same id (a plugin command and a plugin skill both named caveman:caveman) are both drawn as selected, and changing the query leaves a stale row on screen. The suggestion list compares isSelected by item.id and uses item.id as the React key. Fix: remove duplicate ids or key rows by position. Version 2.1.300.
