[Model behavior] PR verdict issued before reading the full comment thread; thread read only after the user challenged twice (recurrence of #101636)

What happened
The user's loaded review procedure requires reading the whole PR thread (body, every comment, reviews, review comments) before any verdict. The model gave a verdict on a PR, and only after the user asked twice whether the full thread had been read did it fetch the full body (including a collapsed reproduce script it had stripped) and the full transcript comment.
Expected
Read the entire thread first, then give one verdict.
Impact
The user has to police a step that is written in the loaded instructions; trust in every verdict drops.
