[Bug] Stray commas added in prompt generation output

Bug Description
Claude asked, "Want me to draft a test-gap comment for this branch, or move on?"
I do not know why Claude persists in adding the stray commas. There should not be a comma before "or". There are a LOT of these that have developed in Claude 5.0 and continue in Claude 5.5. I've had to add aggressive "recheck English grammar for all commas" to all my code review agents, but that doesn't help with parsing sentences that Claude writes in normal chat. In this case, the comma doesn't interfere too much with readability, but there are other cases where they really mess with sentence structure. Please train the models better for comma usage.
