Description

A Bash tool call (gh auth status 2>&1 | head -10) was blocked with "This Bash command contains multiple operations. The following part requires approval", but no approval prompt appeared in the user's session. The user could not approve it, so the model repeatedly told them to "approve the prompt" that did not exist.

Steps to reproduce

  1. Start a Claude Code session in the in-app assistant (spaCR Qt app, macOS, Claude Agent SDK).
  2. Ask the assistant to run a gh command, e.g. gh auth status 2>&1 | head -10.
  3. The tool result is a permission error requiring approval, and no prompt is shown to the user.

Actual behavior

  • The tool call errors out with an approval-required message.
  • The user sees no prompt, and there is no way to approve from their side.
  • The model could not tell whether a prompt had been shown, so it kept telling the user to approve one.

Expected behavior

Either the user gets an approval prompt, or the tool result says clearly that no interactive approval is available in this host. The model could then give the user the command to run themselves instead of looping.

Environment

  • macOS (Darwin 25.6.0), zsh
  • Host: embedded Claude Agent SDK (non-terminal app)
  • Model: claude-sonnet-5-5
