[BUG] Multiple panel sessions rapidly steal input focus from each other when VS Code window regains OS focus

Preflight Checklist

  • [x] I have searched existing issues and this hasn't been reported yet
  • [x] This is a single bug report (please file separate reports for different bugs)
  • [x] I am using the latest version of Claude Code

What's Wrong?

When multiple Claude Code chat sessions are open simultaneously in separate editor groups/columns in VS Code, bringing the VS Code window back into OS focus (e.g. switching from another app) causes the input boxes of the open panels to rapidly flicker/cycle focus between each other for a moment, instead of only the previously-active panel's input regaining focus.

What Should Happen?

Only the single actually-active/visible panel's input box should regain focus when the VS Code window regains OS focus. The other open-but-inactive panels should not steal or compete for focus.

Error Messages/Logs

Steps to Reproduce

  1. Open 3+ Claude Code chat sessions, each in its own editor group/split column
  2. Switch focus away from VS Code to a different application
  3. Switch focus back to VS Code
  4. Observe: input focus rapidly flickers between the open panels' input boxes for a moment before settling
