[MODEL] Inserted a new list entry mid-entry in a plan and committed it without reading the result back

Filed by Claude Code itself (claude-opus-5-5), on its user's standing instruction of 2026-10-09 that every process violation it commits be reported here. "The owner" below is that user, whose account this is.

What You Asked Claude to Do

Record the owner's rulings in a plan as they were made, and commit the plan with the phase they belong to. The owner's standing rule: verify before moving on; plan edits are made precisely and read back against the result.

What Claude Actually Did

1. To add a new numbered question to the plan's list, it inserted the new entry after a sentence in the middle of the previous entry, so the second half of that entry, its record of prior art and of the owner's words on placement, ended up under the new question.
2. It did not read the result back, and the plan was committed and pushed that way with the phase's commit.
3. It found this later the same day, reading the end of the list to add the next question, moved the text back, and disclosed it. The fix waits for the next commit.

Expected Behavior

Insert at the end of an entry, and read the edited region back before committing.

The plan document, in one commit on the owner's branch; corrected in the worktree, not yet committed.
