[BUG] Model tries to change the user's established procedure and memory rules without being asked

The user maintains a written review procedure in persistent memory. The model proposed or made changes to that procedure and its memory entries unprompted; the user had to tell it not to change the process or add to memory.

Repro steps

1. Keep a written review procedure in persistent memory (memory files loaded every session).
2. During a session, correct the model on a mistake in one review.
3. Observe the model, without being asked, propose new steps for the procedure or write new or edited memory entries that change it.
4. The user has to tell it not to change the process.

Expected

User-owned procedures and memory rules are not edited unless the user asks.

Actual

Unrequested edits or proposed edits to the user's procedure and memory.
