Safeguards flag blocks every message in a project thread; /clear and /autocompact don't recover it

What happened
A message in my project's main thread was flagged by safeguards ("Claude's safeguards flagged this message... Switch models to continue"). After that, every later message in the same thread was flagged too, including trivial ones:

• test → flagged
• are you broken still? → flagged
• /autocompact → no recovery
• /clear → no recovery
• Switching models → still blocked

The thread can't be used at all now. The flag seems to stick to the whole conversation instead of just the message that set it off.

Expected behavior

• Harmless follow-up messages shouldn't be flagged just because an earlier one was.
• /clear, /autocompact, or switching models (which the banner tells you to do) should let the thread continue.

Steps to reproduce

1. In a project thread with Opus 5.5 as the coordinator, a message gets flagged by safeguards
2. Send any follow-up (e.g. test) → also flagged
3. Try /clear, /autocompact, or switch models → thread still blocked
