What You Asked Claude to Do

I work on AI adoption and systems design. This is a historical product failure report assembled from my local incident log; an independent controlled replay was not run. The task: build a second round page from a decision tree, following the structure of the existing first round page, which was named as the reference. The private task prompt is not reproduced.

What Claude Actually Did

Claude built the second round page without opening the first round page as a structural reference. The required choices, depth toggles and progress indicator were missing, so the page and its companion had to be built again.

Expected Behavior

Read the named structural reference before generating the next page, keep the requested controls, and check the saved page against the structure it was asked to follow.

Haven't tried to reproduce in an independent clean session. The local log records the historical occurrence.

Ask Claude Code to build a second page that must follow the structure of an existing first page, naming that page as the reference.

Check whether it opened the reference page and whether the required controls are present.
