[Bug] Terminal resize duplicates recent output in scrollback on Windows

Bug Description
On Windows (Claude Code 2.1.295), when the terminal width changes, Claude re-prints recent output from the top of the screen without clearing the previous copy, so the scrollback keeps a copy at each old width. Reproduced in VS Code's built-in terminal.
To repro: get a reply a few paragraphs long, split the terminal (or narrow the window), close the split, then scroll up. With a short conversation the whole conversation is duplicated; in a longer one, the latest turn.
