[Bug] Stray "GOK" appears in prompt at startup in iTerm2 only

Bug Description
Stray "GOK" appears in the prompt at every startup in iTerm2 3.5.12 (3 of 3 launches). Doesn't happen in macOS Terminal.app (0 of 3). Claude Code 2.1.295, TERM=xterm-256color, no tmux, macOS Darwin 24.6.0.
