[Bug] Safeguards falsely block benign requests to explain recent code changes

Bug Description
i keep getting generic blocks on simple requests. i ask things like 'help me to understand the changes you just implemented' and i get a generic response 'Opus 5.5's safeguards stopped the response above' with a generic error report. this is incredibly frustrating. i just want to use claude to better understand the changes you just made.
