What were you trying to do?

In a chat in Claude Desktop (Windows, Max plan), I asked Claude to create an issue in a public repository through the GitHub connector. The connector is connected and authorized: read-only tools from the same connector (for example, fetching my own GitHub profile) work in the same conversation.

What happened instead?

The issue was never created. Each time Claude called the "Create or update issue" tool, the tool call was shown as failed in the chat, with this result: "An interactive form has been shown to the user for creating a new issue ... The issue has NOT been created yet; only the form was rendered. Wait silently for the user to review and click Submit."

The confirmation form did not stay on screen, so I had no chance to review or submit it. This happened three times in a row in the same conversation. On the first attempt a GitHub form appeared briefly and closed by itself. On the second attempt no form appeared at all. On the third attempt, with no title or body pre-filled, the form appeared briefly and closed by itself again.

So the problem does not depend on the pre-filled content, and it is not an authorization problem. It looks like the form is dismissed as soon as the tool result comes back flagged as an error, or the client does not keep the form open after the tool call ends.

Expected: the confirmation form stays open until I submit or cancel it, and the tool call is not shown as failed while it is waiting for me.

Environment: Claude Desktop on Windows, Max plan, GitHub connector, Claude Code 2.1.294 installed on the same machine. No screenshot attached for privacy reasons. This issue was filed through the browser as a workaround.
