[Bug] Unclear safety check blocks on repeated terminal commands with no explanation

Bug Description
Claude can open a clean terminal with reasoning behind it, don't know if claude could then technically bypass it if they wanted to. Safety check block didn't start until second and third use. I don't know if this is the route claude should've taken.
