[Model behavior] Files GitHub issues on subjects it chose itself, one misstating what happened, instead of the ones requested

Summary

The user asked the model to raise a specific set of GitHub issues about its behaviour. The model chose two subjects itself and filed them. One misstated what happened: it called the user's clear approval of a command "ambiguous" and described the model's action as unauthorized. The user had to point out that the filed issues were not what they had asked for.

Environment

  • Claude Code CLI, Opus-class model, 1M context
  • gh authenticated; issues filed in a public repository

Expected

Before filing publicly in the user's name, the model files exactly the issues the user asked for, or asks which ones they mean. Filed content must state what happened accurately.

Impact

Public issues with the wrong subject or inaccurate content, filed under the user's account and needing cleanup.
