Bug Description
In plan mode, Claude Code still asks before it runs an MCP tool that is on my allow list.
Tool: mcp__product-copilot-mcp-server__search_confluence_cloud_content_with_cql
Rule: the exact tool name is in permissions.allow in ~/.claude/settings.json. The JSON is valid. There are no ask or deny rules and no managed settings.
Expected: plan mode uses the allow rule and runs the tool without asking.
Actual: I get "Do you want to proceed?" every time.
The permission-modes docs only say how plan mode handles shell commands. They don't say what happens to MCP tools.
