[BUG] Personal skill listed twice — once unscoped, once as self-namespaced plugin:skill

What's Wrong?

I have a personal skill installed at ~/.claude/skills/swiftui-pro (symlinked from ~/.agents/skills/swiftui-pro, author: Paul Hudson, v1.1). It is not installed via any plugin — I checked ~/.claude/plugins/, ~/.claude/plugins/marketplaces/claude-plugins-official, the project's .claude/ directory, and ~/.claude.json for any second registration and found none.

Despite that, the available-skills listing shows it twice:
• swiftui-pro — Comprehensively reviews SwiftUI code...
• swiftui-pro:swiftui-pro — Comprehensively reviews SwiftUI code... (identical description)

It looks like the listing logic is generating a plugin:skill form for a skill whose name happens to match something, even though it's a plain personal skill with no plugin manifest. Expected: it should appear once. Happy to share more environment details if useful.

What Should Happen?

Expected: swiftui-pro appears once in the list.

Steps to Reproduce

1. Install a personal skill (not a plugin) under ~/.claude/skills/<name> — in my case ~/.claude/skills/swiftui-pro, a symlink to ~/.agents/skills/swiftui-pro, with a SKILL.md containing name: swiftui-pro.
2. Do not install any plugin named swiftui-pro — confirm none exists under ~/.claude/plugins/ or any configured marketplace.
3. Start a Claude Code session in any project (I used Xcode's Claude Code integration, but this is likely unrelated).
4. Ask the assistant what skills are available, or trigger any flow that lists available skills (e.g. the <system-reminder> skill listing shown to the model).
