[BUG] --max-budget-usd is checked after each call returns, so a $1 cap stops at $1.38 (3/3, 2.1.292)

What's Wrong?
claude --help describes --max-budget-usd as "Maximum dollar amount to spend on API calls". In practice the run's cost is compared with the cap only after a call has returned and been priced. The call that crosses the cap is never stopped, so a run can end above its maximum by up to one full call.
Measured on 2.1.292 against a local stand-in for the Anthropic API (no real model, no real spend; Claude Code prices the reported usage with its own table for claude-sonnet-4-5). Three runs per row, all three identical:
| cap | usage per call | calls sent | total_cost_usd at stop (rounded) |
|---|---|---|---|
| $0.50 | 190,000 in / 8,000 out ($0.69) | 1 | 0.69 |
| $1 | 190,000 in / 8,000 out ($0.69) | 2 | 1.38 |
| $1 | 1,000,000 in / 1,000 out ($3.015) | 1 | 3.015 |
| $10 | 1,000,000 in / 1,000 out ($3.015) | 4 | 12.06 |
The $1 / 190k row is the clearest one. After the first call, $0.69 was spent and $0.31 was left. The next request carried at least the same 190,000 input tokens, which is $0.57 of input before any output. Claude Code knew that from the previous call's usage and sent it anyway. The $10 row has the same shape: three calls came to $9.045, $0.955 was left, and a fourth call that would cost about $3 went out.
What Should Happen?
Either of these would match the help text:
- Before sending a request, estimate its cost from the input tokens Claude Code is about to send (it already tracks context size for auto-compact) plus a bound on output (
max_tokens). If that estimate is larger than what is left under the cap, stop witherror_max_budget_usdwithout sending. - If that is out of scope, document the actual guarantee: the run stops once spend has passed the cap, so the real ceiling is the cap plus the most expensive single call. People budget fleets by adding up caps (N workers × $X), and that sum is wrong by up to one call per worker.

Steps to Reproduce
Needs claude and bun. Nothing leaves the machine.
mkdir /tmp/cap && cd /tmp/cap && echo hi > a.txt
cat > loop.ts <<'EOF'
let n = 0;
Bun.serve({ port: Number(process.env.PORT), hostname: "127.0.0.1", async fetch(req) {
const j: any = await req.json().catch(() => ({}));
const names: string[] = (j.tools || []).map((t: any) => t.name);
const tool = names.includes("Read") ? "Read" : names[0];
const content = tool ? [{ type: "tool_use", id: `toolu_${++n}`, name: tool, input: { file_path: `${process.cwd()}/a.txt` } }] : [{ type: "text", text: "OK" }];
const usage = tool ? { input_tokens: Number(process.env.IN), output_tokens: Number(process.env.OUT) } : { input_tokens: 1, output_tokens: 1 };
return Response.json({ id: `msg_${n}`, type: "message", role: "assistant", model: j.model, content, stop_reason: tool ? "tool_use" : "end_turn", stop_sequence: null, usage });
}});

The stand-in answers every request that offers tools with a Read call, so the run only stops at the cap. Control: drop --max-budget-usd and pass --max-turns 2, and it stops with error_max_turns at $1.38, which shows the meter reads the reported usage. Start the server with IN=1000000 OUT=1000 for the $3.015 row.

Additional Information
Related feature requests: #95964 (a per-session cap for interactive sessions) and #85422 (a circuit breaker). An interactive cap built on the same check would inherit this unless it prices the next call before sending it. The one before-the-call dollar check we found across ten harnesses is Qwen Code's batch planner, which refuses to submit when the worst case exceeds maxCostUsd (batch-workflow.ts:284-310). Codex's rollout_budget token limit has the same after-the-call shape (limit 50,000, stopped at 61,500).
