Bug Description
▎ /goal Stop hook loops forever when the last step needs a user-only action ▎ - A /goal condition included "post a message in a Slack thread". The Slack MCP connector needed OAuth sign-in, which only the user can do. ▎ - Claude finished the other steps (PR, Jira) and asked the user to sign in. The goal Stop hook then blocked every stop with the same "message not posted" feedback. ▎ - That repeated for hundreds of rounds, with the same tool check and the same reply each time, until the user came back and signed in. ▎ - Expected: when the remaining step depends on a user action, the hook should pause or end the turn once and wait for the user, not re-trigger in a loop. ▎ - Repro: set /goal with a final step that needs an unauthenticated MCP connector, complete the other steps, then try to end the turn.
